MYCARE is contracted by many clients to provide HomeFront Assessments for patients funded by DVA (Department of Veterans Affairs). Our Registered Nurses visit Patient homes and conduct a comprehensive assessment as per DVA guidelines and principle assessment areas.

 

The DVA HomeFront Program provides patients with funding assisting in the provision of minor home modifications and appliances such as non-slip mats, grab rails and other safety materials and items.

 

The MYCARE Nurse will also provide information concerning other Community and Government services that may be of assistance to each patient based on their individual needs.
