MYCARE collate and process all data collected from our In-Home Health Assessments and develop professional and comprehensive care plans on behalf of the Doctor. Our Nursing Team process information and develop these plans in a professional format facilitating full funding eligibility for our Clinics with minimal Doctor input required subject to Doctor’s determination for involvement.

 

The GPMP will include all patient problems described in detail and options for Doctor to indicate actions for each issue as required. All possible Nursing work and form completion is done providing the Doctor with minimal effort required to complete the plans.

MYCARE is eligible to stand as one of the three team members in the TCA allowing the Doctor to focus Patient consultation in specific areas if suitable.
