All MYCARE Health Assessments are conducted in the Home of the patient. Our Registered Nurses travel to Patient homes and spend an hour with each patient collecting not only the minimal information required by Medicare but additional information we believe essential for a comprehensive assessment of Patients over 75.

 

As well as reviewing ACTUAL living environment and safety concerns, all patients are assessed for BGL, Urinalysis, Hearing, Vision and Oral Health, Smoking and Alcohol status, Diet/Nutrition, BMI, Footcare, Sleep, Transport, Skin Condition and Respiratory Assessment.

 

The MYCARE Health Assessments exceed criteria required for a Prolonged Assessment (Medicare 707) and facilitate full funding eligibility for our Clinics with minimal Doctor input required subject to Doctor’s determination for involvement. Combined with our Care Plan services and eligibility for Medicare items 721 and 723, our Health Assessments provide the most cost effective and comprehensive care package for Patients over 75.
